Illinois residents may need different applications for medical or food benefits, transportation discounts and Medicare counseling. Start with the specific program rather than treating every state service as part of SSI. Local guides below identify additional contacts in Chicago, Aurora and Rockford.
Apply Through ABE or Another Accepted Route
Illinois HFS: Applying For Medicaid explains how to apply for Medicaid online, by telephone, in person or on paper. The ABE application can also be used for SNAP, certain cash programs and Medicare Savings Programs. Select the benefits you want to request and retain the tracking or case number. If the state asks for verification, follow the instructions and response date in the notice.
Check Benefit Access Requirements
Illinois Benefit Access Eligibility describes eligibility for license-plate discounts and the Ride Free Transit Card. Age or qualifying disability, Illinois residency and income requirements apply. The application’s household rules also matter. Read the current criteria and accepted disability documentation before submitting. This state program is distinct from Medicaid and from an operator’s paratransit certification process.
Medicare Counseling Through SHIP
Illinois Senior Health Insurance Program is a statewide health-insurance counseling service for Medicare beneficiaries and caregivers. It provides educational resources and a way to locate assistance in your area. Use it to understand Medicare choices or identify questions to ask about existing coverage. Counseling is different from an eligibility determination for a state assistance program or a federal disability claim.
Before You Contact a Program
- Identify the benefit or coverage question before choosing an application.
- Gather the household information and verification specifically requested.
- Save the ABE tracking number or telephone application case number.
- For Medicare counseling, keep current coverage notices and your questions together.
- Use the relevant local guide for housing, accessible transit or community support.
Federal Benefits and Next Steps
Apply for SSDI[1] or SSI[2] through the Social Security Administration. Local resource programs have separate applications and do not decide federal disability benefits.
For the federal process, read our Disability Application Guide and Document Preparation Checklist.

Workers’ Compensation, Medicare and Veterans Benefits in Illinois
Each program has its own application and decision process. These starting points help you find the agency or counseling program for Illinois.
Workers’ Compensation
The current forms page says the IC01 Application for Adjustment of Claim is completed directly in CompFile. Downloadable copies of that application are marked for reference. Use the live filing instructions rather than older handbook language describing multiple paper copies.
Medicare Counseling and Cost Help
