Rockford residents can start with regional aging support, city housing programs and RMTD paratransit. Housing assistance depends on the program and available funding; accessible transportation has its own functional eligibility process.

Northwestern Illinois Aging and Disability Support

Northwestern Illinois Area Agency On Aging serves Winnebago County and other named counties. It offers help for older adults, adults with disabilities and families, with links to caregiver, nutrition, health and benefits resources. Use the county-specific information or appointment route to explain your needs and ask which service is appropriate.

City Housing Programs and Intake

Rockford Housing Programs describes assistance for people at risk of eviction or unstable housing and separate services for people experiencing homelessness. Programs have their own eligibility rules and depend on grant funding. The Single Point of Entry is a route to assessment for homeless housing services; it is not a promise of an available unit.

RMTD Paratransit Certification

Rockford Mass Transit District Paratransit Eligibility requires an application and certification based on disability-related inability to use regular transit. Its instructions describe completing the application in person and arranging transportation to the office if needed. Age or distance to a stop alone does not establish eligibility. Review the current application and explain the actual barriers you face.

Before You Contact a Program

  • For aging support, identify Winnebago County and the person’s daily needs.
  • For housing, explain the present living situation and any notice received.
  • For paratransit, ask about the application visit and transportation to it.
  • Retain written decisions and follow-up requests from each program.

Federal Benefits and Next Steps

Apply for SSDI[1] or SSI[2] through the Social Security Administration. Local resource programs have separate applications and do not decide federal disability benefits.

For the federal process, read our Disability Application Guide and Document Preparation Checklist.
