Which System Handles This Claim?

Illinois distinguishes the employer or insurer’s handling of an injury from a case filed at the Workers’ Compensation Commission. A commission arbitrator decides contested matters, with review procedures beyond that first decision.

How Do I Report the Injury?

Report the injury to your employer and retain the reporting record. Ask for the insurance administrator and claim number. Do not assume the employer’s accident report means that an Application for Adjustment of Claim has been filed at the commission.

What Do I File?

The current forms page says the IC01 Application for Adjustment of Claim is completed directly in CompFile. Downloadable copies of that application are marked for reference. Use the live filing instructions rather than older handbook language describing multiple paper copies.

What if Benefits Are Disputed?

The forms collection separately lists hearing requests and immediate-hearing petitions. Starting a commission case does not mean every issue is automatically scheduled for hearing. The appropriate request depends on the benefit dispute and procedural posture.

Where Can I Get Help, and What Should I Bring?

The commission links guidelines for employees filing without an attorney. Compare your application, service record, medical reports, and wage documentation before a hearing. For a settlement, use current CompFile guidance as well; the forms page distinguishes settlements involving self-represented workers from cases already assigned a number.
