In Louisiana, health coverage, help with daily activities, and parish-level services have different entry points. Begin with the need you want addressed: paying for medical care, arranging support at home, or finding a local housing or transportation program. This directory helps you choose a starting point without treating one application as approval for another.
Health Coverage Through Louisiana Medicaid
Louisiana Medicaid explains coverage and connects applicants to regional offices and application centers. You can apply online, by telephone, by mail, or in person. If you need application help, choose the office or center for your region. Keep the submission information and any later requests for documents together.
Help at Home and Long-Term Care
Louisiana Medicaid Long-Term Care separates facility care from home and community-based programs. Each program has its own services and eligibility requirements. Ask which assessment and application fit the assistance you need; general Medicaid coverage alone does not establish that a particular home-support service is available to you.
Choose the Right Disability-Service Entry Point
Louisiana Department of Health Disability Guide directs people with intellectual or developmental disabilities to their local human services district or authority for eligibility review. Its guidance separately identifies the Office of Aging and Adult Services for older adults and people with adult-onset disabilities. Describe the person’s age, support needs, and current living arrangement so the agency can direct the request.
Before You Contact a Program
- Identify your parish and the service you need first.
- List daily activities that require assistance, with concrete examples.
- Ask which documents, assessments, and separate applications are required.
- Record the next step and how to check progress; keep copies of notices.
Federal Benefits and Next Steps
Apply for SSDI[1] or SSI[2] through the Social Security Administration. Local resource programs have separate applications and do not decide federal disability benefits.
For the federal process, read our Disability Application Guide and Document Preparation Checklist.

Workers’ Compensation, Medicare and Veterans Benefits in Louisiana
Each program has its own application and decision process. These starting points help you find the agency or counseling program for Louisiana.
Workers’ Compensation
For a denied benefits claim, the official FAQ identifies Form LW-WC-1008, Disputed Claim for Compensation. This begins an administrative proceeding and should not be mistaken for an employer’s initial accident report.
