Minnesota separates health-coverage applications from its MNbenefits application for food, cash, and related supports. Counties and Tribal Nations play a role in reviewing applications and arranging services. Choosing the correct route first can save you from repeating information or waiting on the wrong office.
Choose the Health-Coverage Application
Minnesota Health Coverage Application Guide explains which application matches the household and coverage requested. The route for families and many adults differs from the forms for certain older or disabled applicants and long-term care. Some applications are paper-only. Use the current decision guidance or ask a county, Tribal agency, or application assister instead of choosing solely because you have a disability.
Food, Cash, and Other Basic Needs
MNbenefits lets residents apply for several state-based benefits in one application. A county or Tribal Nation eligibility worker reviews the information and may request documents or an interview. The site also provides document upload and status guidance. It is not a substitute for checking the separate health-coverage instructions.
Disability and Aging Navigation
Minnesota Home and Community Services Guide describes daily-living and community supports. Disability Hub MN offers free statewide help with questions and finding services; Minnesota Aging Pathways helps older adults and caregivers. These are useful starting points when the right program is unclear. Ask whether a local assessment, financial eligibility review, or referral is needed for the support you are considering.
Before You Contact a Program
- List everyone seeking coverage and the benefits each person needs.
- Keep notices, application dates, and county or Tribal worker details together.
- Describe daily support needs separately from the need to pay medical bills.
- Ask which submission route is correct before sending duplicate applications.
Federal Benefits and Next Steps
Apply for SSDI[1] or SSI[2] through the Social Security Administration. Local resource programs have separate applications and do not decide federal disability benefits.
For the federal process, read our Disability Application Guide and Document Preparation Checklist.
