Nevada Workers’ Compensation: Claims, Disputes, and Help

Which System Handles This Claim?
Nevada distinguishes employee notice to the employer on Form C-1 from the actual claim on Form C-4. Completing an accident notice is not the same as confirming that the claim reached the correct administrator.
How Do I Report the Injury?
Notify the employer or supervisor and complete the C-1 notice. Keep a dated copy. For treatment, ask about the authorized provider panel or network; the state FAQ separately addresses emergency care.
What Do I File?
The C-4, Employee’s Claim for Compensation/Report of Initial Treatment, is supplied by the medical provider. Both the employee and medical professional complete their portions. Ask whether the completed form was sent to the correct claims administrator and keep the resulting claim information.
What if Benefits Are Disputed?
The insurer’s written determination should explain appeal rights. The state directs appeals to the Department of Administration’s Hearings Division and distinguishes insurer decisions from managed-care determinations. A complaint about claim handling is separate from the appeal needed to challenge a decision.
Where Can I Get Help, and What Should I Bring?
The official injured-worker page links the complaint process, hearing resources, and the Nevada Attorney for Injured Workers. Keep the C-1, C-4, decision, and proof of receipt. Ask which review route applies before assuming a complaint preserves the appeal; different types of determination have different deadlines.
