Indiana Workers’ Compensation: Claims, Disputes, and Help
On This Page
Which System Handles This Claim?
Indiana’s employee resources distinguish informal assistance, a formal claim for an award, and review by the Full Board. Selecting the right route depends on whether you need help resolving a problem or are challenging a decision in an existing case.
How Do I Report the Injury?
Tell the employer about the work injury and request the insurer or claim administrator’s details. Keep your report and any written response. If the dispute concerns a particular bill or period off work, identify those dates and amounts before seeking assistance.
What Do I File?
The board’s employee page lists Request for Assistance, State Form 45442, for the assistance route. It separately lists Application for Adjustment of Claim, State Form 29109. Read the instructions for the form that matches the action you intend to take.
What if Benefits Are Disputed?
The same page lists Application for Review by Full Board, State Form 1042. That is a review procedure, not the initial injury report. Check the decision and the current filing directions before using it.
Where Can I Get Help, and What Should I Bring?
Bring the employer’s information, insurance correspondence, treatment records, and work-status documents when asking which procedure applies. Keep proof of submission for every form. Informal contact is useful for clarifying a problem, but obtain a clear answer about any formal filing or review deadline that still needs to be met.
