Wisconsin residents can seek food and health coverage, help comparing long-term support options, and employment services through different public systems. County and Tribal agencies are important entry points. Choose the service that fits your immediate need, then confirm who handles your application and what happens after submission.

Apply for Food and Health Benefits

DHS’s ForwardHealth application guide explains ACCESS, the state’s online route for multiple benefit programs. It also identifies phone, paper, and in-person options through county or Tribal agencies. FoodShare applicants must complete an interview, and individual programs request different information. Gather household, income, and coverage details relevant to your application, but ask the agency about missing information rather than guessing. Keep copies of notices and documents you submit.

Find Your ADRC or Tribal Resource Specialist

The statewide ADRC directory connects older adults, adults with disabilities, caregivers, and people planning ahead with local information. ADRC guidance is available regardless of income. Staff can discuss long-term care, benefits, transportation, and other supports, including through a requested home visit. An information consultation does not establish eligibility for a funded service; ask about the assessment and financial requirements of the specific program you choose.

Vocational Rehabilitation and Employment

DVR’s Think Possibilities guide explains support for finding, keeping, or advancing in a job when disability creates employment barriers. You and a counselor choose a job goal and the services in an Individualized Plan for Employment. DVR uses an Order of Selection waiting list when it cannot serve everyone. Ask about current availability and any cost sharing before arranging a service; most services have no charge, but some may require a contribution.

Before You Contact a Program

  • Identify your county or Tribal agency and the specific program needed.
  • Prepare a short description of daily support needs or work barriers.
  • Keep notices, application records, and requested verification together.
  • Request an accessible format or communication support when arranging contact.

Keep Federal Benefits Separate

Community-benefit and employment applications have separate requirements from federal disability claims. SSA’s Disability page[1] explains SSDI; SSA’s SSI overview[2] explains SSI. Apply for federal benefits through SSA. Starting a local service application does not file an SSDI or SSI claim.
