North Dakota combines several economic-assistance applications in its Self-Service Portal, while local human service zones provide county entry points. Adult and Aging Services offers a different route for support at home. Begin by identifying whether you need financial benefits, a service assessment, or help with an existing case.

Apply for Help and Manage a Case

North Dakota Apply for Help provides routes for Medicaid, food, home-energy, and family assistance. You can apply online, request a mailed application, or return the appropriate paper form through a county office. The Customer Support Center handles case questions, changes, status, and documents. Its notices distinguish upcoming 2027 Medicaid Expansion changes from other program rules; use the instructions for your own coverage category.

Find Your County Entry Point

North Dakota Human Service Zone Directory identifies local offices in every county. Human service zones help people access benefits, community supports, and referrals. Use the county listing to locate the appropriate office; the host county’s administrative role does not mean everyone must travel there. Confirm what can be handled through the centralized support center and what requires local contact.

Explore Support at Home

North Dakota Adult and Aging Services Guide connects older adults and adults with physical disabilities with home and community-based services, caregiver resources, and care-planning information. The Aging and Disability Resource Link now points to the Care Choice website. Ask which assessment and funding route match your needs; a resource referral is different from approval for a particular service.

Before You Contact a Program

  • Identify your county and whether the request is new or already in progress.
  • Keep application and document-submission details with current notices.
  • Describe daily assistance needs and the support already provided.
  • Ask about separate service assessments and how to check progress.

Federal Benefits and Next Steps

Apply for SSDI[1] or SSI[2] through the Social Security Administration. Local resource programs have separate applications and do not decide federal disability benefits.

For the federal process, read our Disability Application Guide and Document Preparation Checklist.
