Ohio Workers’ Compensation: Claims, Disputes, and Help
On This Page
Which System Handles This Claim?
Ohio divides responsibilities between the Bureau of Workers’ Compensation, which handles claim determinations, and the Industrial Commission, which hears disputes. The correct destination depends on whether you are starting a claim or challenging an order.
How Do I Report the Injury?
Notify the employer and tell the treating provider the injury or illness is work-related. BWC permits claims to be reported by workers, medical providers, employers, and authorized representatives. Confirm whether anyone has already submitted the report.
What Do I File?
BWC’s filing instructions identify the First Report of Injury, Occupational Disease or Death, or FROI. They describe online, telephone, mail, and fax options. Keep the confirmation and claim number, and ask about the applicable process if the employer is self-insured.
What if Benefits Are Disputed?
The Industrial Commission directs a party appealing a BWC order to Form IC-12 or online filing through ICON. It provides separate information for later hearing levels and appeals of its own decisions.

Where Can I Get Help, and What Should I Bring?
The Office of the Ombudsperson is an impartial resource for unresolved process questions involving BWC or the commission. Have the order, receipt date, claim number, and supporting records ready. A medical-treatment disagreement may follow different preliminary procedures, so identify the actual decision before selecting an appeal form.
