Which System Handles This Claim?

Mississippi’s commission distinguishes a claim being handled by the employer or insurer from a contested case. Its rules identify the employee’s Petition to Controvert and the employer or carrier’s Notice of Controversion as different documents.

How Do I Report the Injury?

Report the work injury to the employer and ask for the carrier or self-insured claims contact. The commission’s rules require workplace coverage information identifying the carrier and the person to whom injuries should be reported.

What Do I File?

To dispute entitlement before the commission, the rules identify the employee’s Petition to Controvert, Form B-5,11. The form calls for the employer, injury, and benefits information. Download the current official form and confirm filing instructions with the commission.

What if Benefits Are Disputed?

A carrier’s Notice of Controversion is its statement that liability is disputed; it is not your petition. A contested case also has later requirements for evidence and setting a hearing. Ask the commission which instructions apply if you are filing without an attorney.

Where Can I Get Help, and What Should I Bring?

Keep the injury report, denial or suspension notice, medical records, and earnings information. The published rules contain provisions with different effective dates, and filing technology can change. This guide does not treat an older rule’s paper-copy directions as a guarantee of today’s submission method. Confirm the current route before transmitting personal information.
