Borderline personality disorder can affect emotional regulation, self-image and relationships. Symptoms and functioning differ across people and over time. A useful disability application describes the documented pattern and support needed without relying on stereotypes or treating the diagnosis as evidence of a person’s character.

SSA Evaluates Medical Findings and Function

Adult listing 12.08 covers personality and impulse-control disorders. It requires specified medical documentation together with the required level of limitation in mental functioning. The areas concern understanding and using information, interacting with others, maintaining concentration and pace, and adapting or managing oneself.

This listing does not have the separate paragraph C alternative found in some other mental listings. If a listing is not met or medically equaled, SSA assesses remaining work capacity, including the effects of other documented conditions. A diagnosis, conflict at work or one crisis visit alone is not the complete test.

Build a Respectful Longitudinal Record

  • Identify diagnostic assessments, therapy records and relevant psychiatric care.
  • Describe the frequency, duration and circumstances of symptoms, including periods of improvement.
  • Explain help with routines, appointments, coping with changes or completing tasks.
  • Include workplace changes, extra support and the reasons an attempted return to work ended, if applicable.

Use concrete observations rather than labels such as “difficult” or “unreliable.” A hypothetical example might describe needing repeated help to resume a task after a stressful interaction. State what happened and what support helped without assuming that all interactions are impaired.

Treatment Is More Than a Medication List

NIMH identifies psychotherapy as the main treatment. Approaches include dialectical behavior therapy and other structured therapies. Medicine is not a first-line treatment for the disorder itself, although clinicians may use it for particular symptoms or co-occurring conditions. Treatment should be planned with qualified healthcare professionals.

Describe participation, benefit, remaining symptoms and actual side effects from medicines used. Explain barriers such as difficulty finding an appropriate therapist, cost or interruptions in continuity of care. Improvement matters, but identify whether intensive support or reduced demands made it possible.

A gap in care needs an explanation, including cost, unavailable appointments or adverse effects. SSA’s failure-to-follow-prescribed-treatment rule applies only in defined circumstances: the person would otherwise qualify, their own medical source prescribed treatment, and they did not follow it. SSA then considers whether it would restore the ability to perform substantial gainful activity and whether there was good cause. A missed visit alone does not answer those questions.

Prepare Forms Around Actual Abilities

Explain concentration, task completion, interaction and adaptation in ordinary settings. Include strengths and activities still possible. If another person helps with forms, identify their role and distinguish their observations from your own account. Do not exaggerate symptoms or use someone else’s history as a model.

If denied, check whether SSA considered the course over time, treatment and support outside a brief examination. Read the decision’s reasons and appeal instructions promptly, and identify relevant evidence that was missing or misunderstood.

Is Borderline Personality Disorder the Same as Bipolar Disorder?

No. They are distinct diagnoses, although symptoms can overlap and conditions may co-occur. The clinician’s assessment and the timing and pattern of symptoms help clarify the record; an applicant should not switch labels to fit a listing.

Applications and Approval Data

This guide concerns adult claims. SSA generally requires a medically established impairment that prevents substantial gainful activity for at least 12 continuous months, is expected to do so, or is expected to result in death. SSDI also has insured-work requirements; SSI has financial and other eligibility rules. Childhood disability rules differ.

Broad mental-disorder figures combine different diagnoses, evidence and decision stages. They do not establish a personal approval probability for borderline personality disorder or measure how deserving someone is of support. See how disability approval statistics are defined.

Use our application guide and medical and work records checklist. If denied, read the notice and its review instructions promptly; our appeals guide explains the process.
