Compassionate Allowances: Expedited Disability Review

Compassionate Allowances, or CAL, help Social Security identify certain serious conditions for expedited disability review. CAL is a processing initiative within SSDI and SSI. It does not replace either program’s eligibility requirements or establish a guaranteed decision date.

Check the Specific Condition

Use SSA’s current condition list and read the complete entry name. Some entries specify a disease subtype, stage, or other characteristic. A broad diagnosis such as cancer is not enough to establish that a particular CAL entry applies.

For example, the list includes some cancer entries qualified by distant metastases, inoperability, or recurrence. Ask your medical provider to clarify the diagnosis in the records rather than choosing a label because it appears on a benefits list. The agency must evaluate the evidence in your case.

Apply Through the Regular Disability Process

There is no separate CAL benefit payment to apply for. SSA identifies possible CAL cases through information in the disability report and through staff review. A potential CAL condition can also be identified after an application has already moved to another review stage.

Tell SSA the accurate condition name and where the supporting records can be obtained. If a relevant diagnosis is made while your claim is pending, report the change and the treating source. Do not assume an earlier application automatically contains a later medical finding.

Prepare Evidence That Identifies the Condition

• List the clinicians and facilities that diagnosed or treated the condition.
• Include available diagnostic reports and relevant treatment dates.
• For cancer, identify the type and stage if known, without guessing.
• Explain where additional records are held and respond to requests for information.

SSA’s application guidance says not to delay filing solely because you cannot gather every document quickly. Provide what is available and explain what remains outstanding.

Keep Medical and Program Questions Separate

SSA’s current CAL instructions require the same disability evaluation process and the same entitlement or eligibility conditions that apply to other SSDI and SSI claims. SSDI work-history requirements and SSI financial requirements still matter. A CAL flag does not establish that every requirement has already been satisfied.

Expedited handling also does not itself waive the ordinary SSDI waiting period or determine the first payable month. Waiting-period exceptions depend on their own rules. Read the award notice for the actual entitlement dates rather than equating a faster decision with immediate payment.

Follow the Claim Through to a Decision

Keep a record of your submission, new evidence, and SSA notices. If you believe a listed condition has been overlooked, identify the exact condition and relevant records when contacting SSA. If a denial arrives, read the reason and appeal instructions. SSA’s CAL guidance provides for priority treatment of denied CAL cases through the appeal stages, but priority is not a promise of a favorable result.
