Which System Handles This Claim?

Hawaii workers’ compensation addresses work-related injuries and illnesses. It is separate from temporary disability insurance for qualifying nonwork conditions. The Disability Compensation Division explains employer coverage requirements and exclusions; identify the correct program before filling out forms.

Official Hawaii Guidance.

How Do I Report the Injury?

Report the injury to your employer immediately. The employer submits its WC-1 report to the division, including when it disputes that the condition is work-related. Ask for a copy and the insurance carrier’s contact information.

What Do I File?

The division’s forms collection includes WC-5, Employee’s Claim for Workers’ Compensation Benefits. Its overview states that an injured employee is entitled to choose the treating physician. Confirm claim and treatment arrangements with the carrier while keeping your medical and work-status records.

What if Benefits Are Disputed?

If a claim is denied or an issue cannot be resolved, either party may request a hearing. The division’s forms page includes the hearing-request form. A decision after that hearing has its own appeal process and deadline, distinct from reporting the original injury.

Where Can I Get Help, and What Should I Bring?

Ask the Investigation Section or the appropriate neighbor-island district office for help if your employer lacks coverage. For other disputes, list the specific benefit requested and attach the carrier’s written explanation. Save the date-stamped decision; a later disagreement about it requires attention to that notice, not just the original accident date.
