Treatment records explain what has been tried, what improved and what still limits everyday activities or work. Use these guides to organize that history and understand the different questions that can arise in an application or a later benefit review.

Choose the Question You Need to Answer

Keep Health Decisions and Benefit Evidence Connected

Describe the care you actually received, the response, side effects and any difficulty obtaining or following treatment. Include what you can still do as well as limitations. SSA considers relevant evidence about symptoms, treatment and functioning; a diagnosis or treatment name alone does not establish disability.

Treatment choices belong with your clinicians. A gap in care is not automatically a denial. SSA’s symptom evaluation considers possible reasons for limited treatment, while its separate failure-to-follow policy has specific requirements, including whether you would otherwise qualify and whether prescribed treatment would be expected to restore work ability.

Start With Your Condition and Your Situation

The condition directory explains condition-specific medical evidence and treatment context. For an initial claim, use the application guidance. If you already receive benefits and have a review notice, start with the continuing disability review guide.

These pages explain how evidence is evaluated. They do not give a personal approval probability or recommend a treatment to obtain benefits. Read the approval-data explainer before interpreting percentages from different programs, years or decision stages.
